    IMPORTANT: If youve made travel reservations in your maiden name, wait until after you get home to start your name change. Your photo ID must match your reservation. If youve already changed your name on your drivers license or passport and your reservation is in your old name, contact your travel provider to make sure your reservation is honored.

    What’s Allowed In Your Legal Name Change

    Your legal name is your first and last name only. The SSA will still print your middle name and suffix on your social security card if enough space is available.

    Your name may only contain letters, spaces, hyphens, and apostrophes. Suffixes are okay. Omit your personal title, such as Mr., Ms., Mrs., Dr., Esq., Sister, or Father.

    Don’t use a nickname or abbreviation, such as Liz for Elizabeth or Deb for Deborah, unless it’s your actual legal name.

    Documents Required For A Name Change

    To begin with all documents given to the Social Security Administration must be original documents or if they are copies, they must come from the issuing agency and certified by that agency. No copies, even notarized copies or receipts for requested copies will be accepted by the SSA.

    Your documentation should prove your legal name change, your U.S. citizenship and your identity.

    If For Any Reason You Cannot Produce These Documents You May Use:

    Gather All Required Documents For Identification

    This is likely the most difficult part of the entire process. Once you complete the SS-5, you will need to gather the required documentation to process your request. As previously mentioned, you will need documents proving both your identity and the changing of your name. One very important item to note is that the documents must be originals or certified copies by the issuing agency. Photocopies or even notarized copies of originals are not acceptable.

    Here are some of the most common documents that are accepted by the SSA:

    Elect To Go Back To Your Previous Last Name

    If you elected to legally change your last name due to marriage or conjugal relationship, you can change your name back to your previous last name by completing an Election to Resume Former Surname – Form 2. This service is only available if you legally changed your last name using the discontinued Election to Change Surname service.

    When you legally change your name back to your previous name, you get a certificate of name change and a new birth certificate . You can use these documents to update other government documents .

      Setup A Marriage Ceremony

      A Marriage Ceremony is inexpensive when done by the state. Most states charge between $25-$60 dollars when performed by a Marriage Officiant. A Marriage Ceremony must be done within 30 days to 90 days, depending on the state, from the marriage license date of issuance.

      Where? When a marriage ceremony is done by the state, its typically performed in the county clerks office. Nothing fancy, but its inexpensive and gets the job done.

      The qualified person performing the marriage ceremony must return the original marriage license to the County Clerks office within the state required timeframe, which is usually within 10 business days. The county recorders office will officially process and record the marriage.

      These documents are the most important part, and you can’t move on to step two without them. When you’re applying for your , ask if you can purchase certified copies in advance.       1. Get your marriage certificate

      The first thing you need to do is get a physical copy of your marriage certificate.

      A marriage certificate is a legal document that verifies your marriage occurred at a specific date and location. Youll need to use it many times during your name change.

      Your wedding officiant should send you the original in the mail two to three weeks after your marriage.

      Youll probably want to use a certified copy for all the name change paperwork. This way, you can keep the original safe at home.

      A certified copy of your marriage certificate is an official duplicate of the original certificate issued directly from the county or courthouse where you received your marriage license. It has special seals to verify its authenticity you cant just make your own copy.

      Luckily, getting a copy of your marriage certificate is straightforward. Simply call the office that issued the certificate and order one or more copies.

      You will have to pay a small fee. Most offices will be able to process your request and send you a certified copy within five to seven business days.

      Note, a marriage certificate is not the same as a marriage license. A marriage license shows that you are legally eligible to marry. A marriage certificate functions as a record-keeping document, proving that you have completed and registered your marriage.
