In normal times, the best ways to apply for a replacement card are to visit the Social Security Administrations website, go to a local SSA office or apply over the phone. However, due to the COVID-19 pandemic, all local Social Security offices were closed to the public for in-person service starting March 17, 2020. The SSA no longer allows you to order a replacement card online or over the phone.

Currently, the only way to apply for a replacement card is to fill out an application and mail it to your local SSA Field Office, along with one proof of your identity, such as your drivers license, U.S. Passport, U.S. birth certificate, U.S. Military ID, or your last Physicians report showing your date of birth.

Once you receive your replacement card, dont put it in your purse or wallet. Keep it stored securely in a safety deposit box, or at home.

Complete The Right Applicationa Form Ss

The Social Security Administration uses an application called the SS-5 for all new and replacement Social Security cards. Even if you move to another state, the application form itself, and the application process, does not change. The SSA is the issuing agency for this form, and will be the ones to review it.

Before you replace a lost, frayed, or existing card, youll need to fill out the SS-5 application. The SS-5 can also be used to apply for an original Social Security card or update information to an existing card.

To get a replacement Social Security card in Massachusetts, go online to the specific SSA website for the SS-5. You will notice the form says SS-5 and Social Security Administration in the upper left corner of all five pages.

Use only a black or blue ink pen to fill out the SS-5. You will begin with your legal name and fill out the information as requested.

Who Should Report Naturalization To Social Security

Ill say that just about everyone should update their social security record with the SSA.

Of course, if you were born a U.S. citizen this would not apply to you so Im just referring to immigrants who were previously permanent residents.

When you notify SSA about your naturalization, they will update their database to show you as a U.S. citizen instead of a lawful permanent resident.

They may also update your social security card so that it no longer has Valid with work authorization only written on it.
