Citizenship

If you have not already established your U.S. citizenship with us, we need to see proof of U.S. citizenship. We can accept only certain documents as proof of U.S. citizenship. These include:

  • U.S. passport

We require this documentation only if you haveNOT already established U.S. citizenship with us.

Name Change

If you need to change your name on your Social Security card, you must show us proof of your legal name change. Documents Social Security may accept to prove a legal name change include:

  • Certificate of Naturalization showing the new name or
  • Court order approving the name change.

If the document you provide as evidence of a legal name change does not give us enough information to identify you in our records or if you changed your name more than two years ago , you must show us an identity document in your prior name . We will accept an identity document in your old name that has expired.

If you do not have an identity document in your prior name, we may accept an unexpired identity document in your new name as long as we can properly establish your identity in our records.

Identity

We can accept only certain documents as proof of identity. For example, as proof of identity Social Security must see:

  • U.S. driver’s license
  • State-issued non-driver identification card or
  • U.S. passport.
  • Employee identification card
  • Health insurance card or
  • U.S. military identification card.

If you have lost a birth certificate, it may take a bit longer to get a replacement. You will need to write or visit the “Registry of Vital Records and Statistics” in the state where your child was born.

In Massachusetts, this office is in Dorchester and charges $18 if you apply in person and $28 if you apply by mail. You can find your child’s birth state office information by searching for “replace birth certificate” online.

How To Apply For A Social Security Number

The easiest way to apply for a Social Security number for your child is to complete a birth registration form, which has a box you can check to request a number for your child. To complete the form, you will need to provide both parents’ Social Security numbers.

For most new parents, it’s easy to obtain the birth registration form, because hospitals usually distribute them while the mother is still a patient.

Additional Document Required For International Students And Foreign Exchange Visitors

International Students or Foreign Exchange visitors are able to work on a part-time basis and must bring additional documentation to prove their status. For J Visas, the DS-2019 Certificate of Eligibility for Exchange Visitor Status is required.

For international students, the most recent I-20 Certificate of Eligibility for Non-immigrant Student Status is required.

Original documents are required to obtain a social security card. Photocopies and notarized copies will be rejected.
