Adoption

Copies Of Documentation Are Not Accepted

Once you have completed the form, along with original copies of documents proving your legal name change and identity, you must mail or hand-deliver it to your local Social Security office.

If you are a U.S. citizen, you must also show proof of your U.S. citizenship, if it is not already on record.

In this regard, all documents must be originals or copies certified by the issuing agency, as photocopies or notarized copies are not acceptable.

To prove your legal name change, you must show one of the following documents:

– Marriage document.

– Certificate of naturalization with the new name.

– Court order of name change.

Now, as proof of identity, it is necessary to provide a government-issued photo ID that has not expired, such as:

– A U.S. passport.

– A driver’s license.

– A state-issued identity document.

It should be noted that if you do not have one of these documents, Social Security staff may also accept a school, work or military identification card or a health insurance card.

Receive A Replacement Social Security Card In Your New Name
