How to replace a lost or stolen social security card.

If you have never been issued a Social Security card, you may apply for one at no cost at a local Social Security office. You will need to:

  • Bring a completed Form SS-5 and,
  • Provide at least two documents to confirm your age, identity and citizenship or legal presence.

Acceptable documents include but are not limited to an original birth certificate, valid U.S. driver’s license, state-issued identification card or U.S. passport. If a U.S. birth certificate exists you MUST submit it. See Learn What Documents You Need to find acceptable documents. Only originals or copies certified by the agency that issued the document will be accepted. Individuals who are not U.S. citizens must provide a current U.S. immigration document and foreign passport with biographical information or photograph.

You can use a my Social Security account to request a replacement Social Security card online if you:

  • Are a U.S. citizen age 18 years or older with a U.S. mailing address
  • Are not requesting a name change or any other change to your card and
  • Have a driver’s license or a state-issued identification card from Michigan or one of the other states listed.

What Original Documents Do I Need

Citizenship

If you have not already established your U.S. citizenship with us, we need to see proof of U.S. citizenship. We can accept only certain documents as proof of U.S. citizenship. These include:

  • U.S. passport

We require this documentation only if you haveNOT already established U.S. citizenship with us.

Name Change

If you need to change your name on your Social Security card, you must show us proof of your legal name change. Documents Social Security may accept to prove a legal name change include:

  • Certificate of Naturalization showing the new name or
  • Court order approving the name change.

If the document you provide as evidence of a legal name change does not give us enough information to identify you in our records or if you changed your name more than two years ago , you must show us an identity document in your prior name . We will accept an identity document in your old name that has expired.

If you do not have an identity document in your prior name, we may accept an unexpired identity document in your new name as long as we can properly establish your identity in our records.

Identity

We can accept only certain documents as proof of identity. For example, as proof of identity Social Security must see:

  • U.S. driver’s license
  • State-issued non-driver identification card or
  • U.S. passport.
  • Employee identification card
  • Health insurance card or
  • U.S. military identification card.

Items You Need To Apply For A Us Passport

  • Application Form – To complete the application for a passport book, card, or both together, first do one of the following:

  • Fill out the application online. Use the Passport Application Wizard to step through the process of filling out the application. When youve completed the application, print it out to take to the passport acceptance facility. Passport applications cannot be submitted online.

  • Fill out the application by hand. Download the passport application form and instructions and complete the application by hand, or go to a local passport acceptance facility to get a copy to fill out.

  • Whether you fill it out online or by hand, it is important that you do not sign the form until you are instructed to do so in front of a passport designated official you will meet when applying in person.

  • Personal documents – Youll need to bring all of the following:

  • An original proof of citizenship document

  • An acceptable photo ID document

  • A photocopy of the front and back of the citizenship document and photo ID document

    You Can Apply For A Social Security Card Replacement Online

    Applying for a card replacement online through your my Social Security account is usually the most convenient option. But you can only use the online application if you dont need to make any changes, such as updating your name. You also have to be an adult with a drivers license or a non-driver ID card and live in one of the more than 40 states and Washington, D.C., that verify state-issued documents for the Social Security Administration.

    Passport Book And Passport Card

    Depending on where you are traveling outside of the U.S., you will need to determine if you need a passport book or a passport card.

    • A passport book is required for all international travel by air.

    • To enter the United States at land border crossings or sea ports-of-entry from Canada, Mexico, the Caribbean, and Bermuda, you can use a passport book or a passport card.

    • A passport card is more convenient and less expensive than a passport book, but it cannot be used for international travel by air.

    What You Will Need To Take With You

    If you are seeking a new card, you must present proof showing your age, identity, work eligibility and U.S. immigration status. If you are seeking to renew a card , you must present recently issued documents that prove your identity, and if born outside the U.S., proof of U.S. citizenship or lawful alien status.     The Social Security Administration introduced the expansion of online services for residents of Minnesota available through its my Social Security portal at www.socialsecurity.gov/myaccount. Kilolo Kijakazi, acting commissioner of Social Security, announced that residents of Minnesota can use the portal for many replacement Social Security number card requests. This will allow people to replace their SSN card from the comfort of their home or office.

    I am pleased to offer Minnesota residents the safe, secure, and more convenient option for replacing their Social Security card through the my Social Security portal, Kijakazi said in a press release. Minnesota residents can now replace their Social Security card online, from the convenience of their home, alleviating the need to send in important identifying documents to the office.

    U.S. citizens age 18 or older and who are residents of Minnesota can request a replacement SSN card online by creating a my Social Security account. In addition, they must have a U.S. domestic mailing address, not require a change to their record , and have a valid drivers license, or state identification card in some participating states.

    Minnesota is one of the many states, including the District of Columbia, where this option is available. Social Security employees are working remotely to provide the vital services the public relies on and the agency currently provides multiple service delivery options for all Americans.

    Types Of Social Security Cards
