When you visit a Social Security office to apply for a Social Security card, take the following papers with you:

  • Your passport with your Machine-Readable Immigrant Visa or Permanent Resident Card if you have it and,
  • your birth certificate and a birth certificate for each member of your family applying for a Social Security number.

If you are applying for a replacement card you will only need your passport with your MRIV or Permanent Resident Card , if you have received it.

A Social Security representative will help you complete the application. You should receive your SSN card within 2 weeks after we have everything we need to process your application. If we need to verify any document you show us, it may take longer.

Here are other actions you should take when your Social Security card has been lost or stolen.

Step 1. Place a fraud alert You should place a fraud alert on your credit file to protect yourself from identity theft.

You can do this easily by calling one of the three major credit-reporting agencies , says Danielle Kunkle, vice president at Boomer Benefits, in Fort Worth, Texas. Once you request a fraud alert, the credit reporting agency is obligated to contact the other two agencies to notify them as well.

Here’s how to complete the 18 items on the form:

  • Provide your first name, full middle name, and last name as you want them shown on the card. If you had a different name at birth or previously went by another name, provide those here, as well.
  • If you previously had a Social Security number, write it here. If not, leave this section blank.
  • Enter the city and state or foreign country where you were born.
  • Provide the month, date, and year you were born.
  • Check the appropriate box to indicate whether you are a U.S. citizen, legal alien allowed to work, legal alien not allowed to work, or other.
