Social Security must verify a birth record for all U.S.-born applicants who apply for an original SSN. An exception is made for a parent who applies for a baby’s SSN at the hospital when the baby is born. To verify a birth record, we will contact the office that issued it.

How Do You Replace Your Social Security Card?

The next step is to find original documents youll need to ensure you have the right information. Remember, you are not applying for a new Social Security card but replacing an existing one.

The SS-5 is used for a lot of different purposes, but to replace your Social Security card in Massachusetts you will need documentation to prove your identity and date of birth. Examples of such documents for U.S. citizens include:

    What Is A Temporary Social Security Card

    If an American citizen has lost their Social Security card, they can apply for an immediate temporary replacement as a placeholder for all official documentation. A temporary Social Security card is only valid for 30 days, and during this time, you must apply for a Social Security card.

    Once you have your new Social Security card, you can shred your temporary Social Security card and use the permanent one for official applications.

    If you’re replacing your Social Security card because it’s been stolen, there are several precautions you should take immediately to reduce your risk of identity theft.
