If you legally change your child’s name because of adoption, court order or any other reason, you need to tell Social Security. If your child is working, also tell your child’s employer. If you do not tell us when your child’s name changes, it may:

  • Delay your tax refund or your child’s refund and
  • Prevent your child’s wages from being posted correctly to your child’s Social Security record, which may lower the amount of your child’s future Social Security benefits.

Yes, you can request a replacement Social Security card online if you are a U.S. citizen age 18 or older with a U.S. mailing address. You must also have a driver’s license or a state-issued identification card from one of these states. Response: We have not made any changes to the proposed rule in response to this comment. Section 422.107 of our current regulations discusses what evidence we will accept to verify alien status, specifically evidence issued by the Immigration and Naturalization Service in accordance with that agency’s regulations. Usually, an SSN card applicant must submit the document with the highest value for proving identity. We call this a primary identity document. In most cases, for a U.S. citizen, this is a State-issued drivers license. If the primary identity document is not available, we would then allow a person to submit an original secondary form of identity.

SSA is temporarily allowing people who elect to apply by mail to submit an original or certified secondary identity document with their paper Social Security Number application if they cannot request a replacement SSN card online. Acceptable secondary level identity documents must meet the rigorous document evaluation standards for authenticity, and have biographical data such as date of birth, age, etc. If you choose to visit a field office for an in-person interview, primary evidence is still required when available.

Step 1. Place a fraud alert You should place a fraud alert on your credit file to protect yourself from identity theft.

You can do this easily by calling one of the three major credit-reporting agencies , says Danielle Kunkle, vice president at Boomer Benefits, in Fort Worth, Texas. Once you request a fraud alert, the credit reporting agency is obligated to contact the other two agencies to notify them as well.

Shows your name and Social Security number with the restriction, “NOT VALID FOR EMPLOYMENT”.

We issue it to people from other countries who:

  • are lawfully admitted to the United States without work authorization from DHS, but have a valid non-work reason for needing a Social Security number or
  • need a number because of a federal law requiring a Social Security number to get a benefit or service.

  • Fill in the parent/mothers name at birth along with her social security number
  • Fill out the parent/fathers name at birth along with his social security number
  • Answer whether the person from question one has ever received a Social Security Card before . If yes, go to question 12. If no, skip to question 14
  • Fill out the name shown on the individuals most recent Social Security Card
  • Write todays date DD/MM/YYYY

    To change your name on a Social Security card, you must submit proof of your identity — this can be either a primary or secondary document described above — and documentation showing your name change. This can include a , a court order approving the name change, a divorce decree or a Certificate of Naturalization.

    You may be able to get away with sending the Social Security Administration just one document if it shows your previous name and your age, birth date or your parents names. This information then must match your Social Security records.

    Again, any documents you mail to the Social Security Administration will be sent back to you.
