Here’s how to complete the 18 items on the form:

  • Provide your first name, full middle name, and last name as you want them shown on the card. If you had a different name at birth or previously went by another name, provide those here, as well.

That being said, you can request a Social Security card very easily online, as its an option available in most states. To do this, you will have to log into your existing account and, if you dont have one, you will need to create a Social Security Account.

    But although this option tends to be a favorite, you need to be aware that a few states dont allow you to apply online. As such, you will not be eligible for online application if you are from:

    • Alabama
    • Oregon
    • Ohio

    Furthermore, you will not be allowed to apply online if you got your identification card or drivers license from a U.S. territory. These include Guam, American Samoa, Puerto Rico, the U.S. Virgin Islands or the Northern Mariana Islands.

    As long as you are not requesting a change of your name to your card, and if you are 18 with a U.S. mailing address and have an I.D. card or drivers license issued from one of the many states or the District of Columbia, you can apply online.

    Also, you should be aware of the fact that its not always that you need a card replacement. If you know your number, that may be enough for you in some cases. However, if you find yourself in a situation where a card is necessary, then you need a replacement, and applying online is very easy.

    Yes, you can request a replacement Social Security card online if you are a U.S. citizen age 18 or older with a U.S. mailing address. You must also have a driver’s license or a state-issued identification card from one of these states.     You will receive your new SSA card in the mail in approximately 10-14 business days, along with your documents if you mailed them in.
